I {name}, hereby apply for training in the martial arts with American Top Team Martial Arts School. Upon acceptance, I pledge to obey all school rules and regulations. I acknowledge that these rules are for the purpose of maintaining order in the dojo and to help protect the students from injury. In addition, I also recognize that risk is involved in the practice of any martial art and accordingly assume that risk.
In consideration of my acceptance into this school, I (including the undersigned parent/guardian if younger than 18 years of age), hereby for myself, my heirs, my executors and administrators, waive and release any and all rights I may have against this martial arts school, Jerry Heath., his family, his instructors, his students, dojo members, employees, agents and authorized guests from any and all responsibilities, liability claims, judgments or demands for damages from accidents or injuries that may arise or may be incurred by the above named applicant while practicing martial arts.
In addition, I (including the undersigned parent/guardian if younger than 18 years of age), hereby for myself, my heirs, my executors and administrators, waive and release any and all rights I may have against American Top Team, its owners, landlord, tenants, leasing agents or anyone connected with the location or building from any and all responsibilities, liability claims, judgments or demands for damages from accidents or injuries that may arise or may be incurred by the above named applicant while practicing martial arts.
Having read and understood the above, I agree to participate in the activity at American Top Team.
